Adult Day Program PSW/ Activation Coordinator- 2 Vacancies

As our programs and services are growing across the region we are looking for two (2) PSW's to join our team!  As this is a blended role; the successful candidate must have experience implementing activities for persons living with a diagnosis of dementia.


The Personal Support Worker will provide personal care, escorting & transferring, meal preparation, assistance with the ADP programs.  

The Activation Support staff will employ activation activities to help persons living with dementia remain active in their homes and in group settings. Using games, sports, crafts and more, they will support families maintain the persons living with dementias physical and emotional well-being. In doing this, they also help patients integrate into their community via group activities. The Activation Support staff will be a part of the client’s circle of care and will support in:

• Supporting families with the transition into long term care

• Developing an in-house recreation therapy program for clients to provide activation and respite

• Provide education for families, staff, and the public regarding enriching recreational activities for persons living with dementia

• Market the recreation program

• Evaluate program at specified intervals

Essential Duties and Responsibilities

·         Monitor participant behaviors & concerns.

·         Work cooperatively within the caregiving team.

·         Monitor participants throughout the day, assist participant with bathroom routines when required     and support mobility needs.

·         Calmly & professionally react to crisis situations.

·         Provide support to co-workers, students and volunteers.

·         Monitor, clean and maintain the work environment.

·         Assist with implementing planned activities and in encouraging participation.

·         Collect, maintain and report required quantitative and qualitative data to support province-wide    monitoring, evaluation and reporting.

·         Assist clients with activities of daily living, including eating, toileting, mobility and reminder to take        their medication.

·         Prepare meals and snacks according to specified diet; serve and clean after meals and snacks.

·         Assists participants upon arrival and departure, helping them with their outerwear and belongings,        including safe storage of medication.

 Activation Role

·         Receive referrals to the program from community partners; NELHIN Home and Community        Care, First Link Care Navigators, BSO

·         Effectively participates in care conferences; assessments of clients and determine eligibility             and appropriateness to the activation program and support inter disciplinary team to              implement the activation care plan

·         Conduct in-house programs with clients as needed based on referral to LTC

·         Follow established job routines on a daily basis.

·         Ensures that care provided is documented as required in a timely manner using the               assigned documentation tools

·         Provide education to families and/or in-home care providers regarding appropriate                       activities for the client.

·         Evaluate the effectiveness of the programming for the individual.

·         Assist with transition from client’s home to long-term care homes as necessary

·         Provide education to a variety of target groups regarding activation for persons with                  dementia, which may include making public presentations.

·         Act as a resource for inquiries from the general public regarding recreation/activation.

·         Maintain program statistics and submit monthly.

·         Contribute to internal and external communications

·         Adhere to program standards and Alzheimer Society policies and procedures

·         Assume responsibility for personal upgrading in knowledge of Alzheimer’s disease, and    ongoing professional development.

·         Participate in fund raising and other team development activities.

·         Assume other functions as may be assigned by the Executive Director.


Other Duties:

·         Perform other duties consistent with the job classification, as required.


Job Qualifications


·         Graduate of an approved Personal Support Work program (by the Ministry of Training, Colleges and Universities) with experience in recreation activities for persons living with dementia.


·         Two (2) years’ experience in long –term care with clients e.g. day programs or residents in a long-         term care home setting

·         Experience working directly with people living with Alzheimer’s disease or other dementias and    their care partners.

·         Strong knowledge of client-centered philosophy.

·         Experience in assessment and care planning/coordination.

·         Experience working in settings requiring inter-professional collaboration.

·         Knowledge of Alzheimer’s disease and related dementias (ADRD).

Other Knowledge, Skills, Abilities or Certifications:

  • Excellent communication (verbal and written)
  • Exceptional interpersonal skills, including shared decision-making and facilitation
  • Ability to take initiative and be resourceful
  • Excellent problem-solving and change management skills
  • Proficiency in technology (e.g.: Microsoft office and case management and care coordination systems)
  • Demonstrated ability to work independently and within a team
  • Expertise and experience in cultural sensitivity and diversity
  • GPA/ U-First and Dementiability Certification preferred


Other Requirements:

·         Bilingualism (English/French)

·         A valid driver’s license and access to a reliable vehicle is required

·         Criminal Records Verification (within 2 months), incl. Vulnerable Sector Screen

·         Valid Ontario Driver’s License

o   Travel across Chapter District as required.
